Topic: Booking a Doctor's Appointment
Receptionist: Good morning. Green Park Medical Centre. How can I help you?
Patient: Good morning. I would like to book an appointment with a doctor, please.
Receptionist: Of course. Is this your first appointment at our centre?
Patient: Yes, it is. I moved to this area last month.
Receptionist: No problem. I need to take a few details first. What is your full name?
Patient: My name is Bilal Ahmed.
Receptionist: Could you spell your surname, please?
Patient: Yes. A H M E D.
Receptionist: Thank you. What is your date of birth?
Patient: The ninth of April, nineteen ninety-eight.
Receptionist: Thank you. And what is your address?
Patient: It is flat 6, 24 River Street.
Receptionist: Is that River Street, near the railway station?
Patient: Yes, that is right.
Receptionist: Can I have your phone number?
Patient: Yes. It is zero seven eight five four, two one six, double three nine.
Receptionist: Let me repeat that. Zero seven eight five four, two one six, double three nine.
Patient: Yes, correct.
Receptionist: Thank you. What is the reason for the appointment?
Patient: I have had a sore throat and a cough for three days.
Receptionist: Do you have a high temperature?
Patient: No, not really. I just feel tired.
Receptionist: Okay. We have an appointment tomorrow morning at ten twenty.
Patient: Tomorrow at ten twenty is fine.
Receptionist: The appointment will be with Doctor Helen Carter.
Patient: Could you spell the doctor's surname, please?
Receptionist: Yes. C A R T E R.
Patient: Thank you. Do I need to bring anything?
Receptionist: Please bring photo identification and a list of any medicines you take.
Patient: I am not taking any medicine at the moment.
Receptionist: That is fine. Please arrive ten minutes early to complete a short registration form.
Patient: Okay. So I should arrive at ten ten.
Receptionist: Yes, exactly. The medical centre is on the ground floor. Use entrance B.
Patient: Entrance B. Got it.
Receptionist: Your temporary patient number is P seven four two one.
Patient: P seven four two one. Thank you.
Receptionist: You are welcome. Goodbye.
